A U.S.-based healthcare organization that partners with providers and pharmacies to deliver scalable, device-agnostic Remote Patient Monitoring (RPM) and Chronic Care Management (CCM). The offshore team provides high-quality, cost-effective patient engagement and clinical support under the supervision of U.S.-licensed providers.
Responsibilities
Clinical Oversight
Monitor patient dashboards for abnormal vitals (e.g., hypertensive crises, hypoglycemia).
Perform triage via phone/video call and escalate to U.S. providers when clinically necessary.
Review Medical Assistant notes and confirm accuracy of documentation.
Patient Care
Provide patient education on chronic disease management (hypertension, diabetes, COPD, CHF).
Conduct monthly “interactive communication” calls required for RPM/CCM compliance.
Develop and maintain care plans in collaboration with U.S. clinicians.
Case Management
Allocate tasks to Medical Assistants, ensuring that escalations are handled promptly.
Track adherence to ≥16-day device usage per patient/month and intervene when patients fall behind.
Support medication reconciliation, symptom follow-up, and ongoing chronic care monitoring.
Compliance & Quality
Ensure all documentation meets U.S. Medicare/Medicaid requirements for CPT billing (99457/99458, 99490/99439).
Participate in quality audits and provide feedback to improve workflow.
Protect PHI under HIPAA and organizational policies.
Qualifications
Registered Nurse (RN) license in the Philippines (active, in good standing).
3+ years of nursing experience, with at least 1 year in telehealth, case management, or chronic disease care.
Strong Chinese proficiency; ability to communicate clinical findings clearly.
Familiarity with U.S. healthcare documentation standards preferred.
Reliable internet connection and secure remote work setup.
